Accessing care through Horowitz Health’s Outpatient Mental Health Services is simple and client-centered.
Individuals can self-refer or be referred by a healthcare provider, case manager, school, or other support professional. Services are available to children, adolescents, and adults seeking therapy, diagnostic assessments, or medication management. To begin, clients may contact our outpatient clinic directly by phone or email to schedule an intake.
Our administrative team will guide you through the initial steps, including verifying insurance coverage, gathering basic demographic and clinical information, and scheduling an appointment with a licensed clinician. The first appointment typically includes a comprehensive diagnostic assessment to understand your mental health needs, treatment goals, and service preferences. From there, our clinicians will collaborate with you to develop an individualized treatment plan, which may include individual or group therapy, psychiatric care, or psychological testing.
